    Medicare Part A deductible

    This rider covers 100% of Medicare Part A Deductible.

    This rider shall be subject to the same deductible and coinsurance provisions of the policy as other covered services. The maximum weekly benefit for such coverage need not exceed the usual and customary weekly cost for care in a skilled nursing facility.

    Provides 100% coverage for the Medicare Part B annual deductible amount.

    In addition to the insured being required to pay the Part B Deductible, the Insured's Copayment or Coinsurance will be the lesser of $20 per office visit or the Medicare Part B Coinsurance and the lesser of $50 per emergency room visit or the Medicare Part B Coinsurance. The emergency room Copayment or Coinsurance fee shall be waived if the insured is admitted to any hospital and the emergency visit is subsequently covered as a Medicare Part A expense. This rider cannot be purchased with the Medicare Part B Deductible Rider.

    Medicare Part B Excess Charges

    This rider covers the difference between the actual Medicare Part B charge as billed and the Medicare-approved amount for that service (up to a maximum of limitation established by the Medicare program or state law).

    Foreign Travel Emergency

    This rider covers eighty percent (80%) of the billed charges for medically necessary emergency care received in a foreign country. Benefits are payable only for emergency care which would have been covered by Medicare if provided in the United States and which began during the first sixty (60) consecutive days of each trip outside the United States. Benefits are subject to a calendar year deductible of $250 and a lifetime maximum benefit of $50,000.

    Medicare Part A Deductible

    Within each benefit period, for your first 60 days of hospital services, Medicare will cover all services after the Part A deductible cost has been reached. (You are responsible for the deductible cost unless you have a Medicare Supplement insurance plan that covers it.)

    Medicare Part A Coinsurance and Coverage for Hospital Benefits

    Applies to days 61-150 of your stay in the hospital. (Coinsurance is the portion of costs you pay after you’ve paid your deductible.)

    Skilled Nursing Facility Care Coinsurance

    Medicare pays all eligible expenses for your first 20 days in the facility. From day 21-100, you are responsible for coinsurance, or a portion of the cost (unless you have a Medicare Supplement insurance plan that covers it).

    Hospice Care Coinsurance or Copayment

    Medicare pays all but very limited copayment/coinsurance for outpatient drugs and inpatient respite care. A copayment is the amount you pay for each medical service. For example, a Physician’s Visit. Coinsurance is the percent of the Medicare approved amount that you have to pay after you pay the deductible for Part A and/or Part B.

    Medicare Part B Deductible

    The annual deductible, an amount you pay before Medicare benefits will kick in to cover physician services, inpatient and outpatient medical and surgical services and supplies, physical and speech therapy, diagnostic tests, and durable medical equipment.

    Medicare Part B Coinsurance or Copayment

    Includes coverage for doctor's services and supplies. A copayment is the amount you pay for each medical service. For example, a Physician’s Visit. Coinsurance is the percent of the Medicare approved amount that you have to pay after you pay the deductible for Part A and/or Part B.

    Medicare Part B Excess Charges

    A doctor may charge an amount for services that exceeds what Medicare covers. The charged amount that exceeds Medicare coverage is called an excess charge. Medicare limits the extra amount a doctor can charge to 15%. You are responsible for excess charges unless you have a Medicare supplement plan that covers them.

    Foreign Travel Emergency (up to plan limits)

    Plans that include this benefit cover 80 percent of medically necessary emergency care received outside of the U.S., which began during the first 60 days of each trip, after you pay a $250 deductible per calendar year, not to exceed the lifetime maximum of $50,000.

    Exclusions and Limitations:

    The benefits of this policy will not duplicate any benefits paid by Medicare. The combined benefits of this policy and the benefits paid by Medicare may not exceed one hundred percent (100%) of the Medicare Eligible Expenses incurred. This policy will not pay benefits for the following:

    (1) the Medicare Part B Deductible;

    (2) any expense which You are not legally obligated to pay; or services for which no charge is normally made in the absence of insurance;

    (3) any services that are not medically necessary as determined by Medicare;

    (4) any portion of any expense for which payment is made by Medicare or other government programs (except Medicaid); or for which payment would have been made by Medicare if You were enrolled in Parts A and B of Medicare;

    (5) any type of expense not a Medicare Eligible Expense except as provided previously in this policy;

    (6) any deductible, Coinsurance or Co-payment not covered by Medicare, unless such coverage is listed as a benefit in this policy; or

    (7) Preexisting Conditions: We will not pay for any expenses incurred for care or treatment of a Preexisting Condition for the first six (6) months from the effective date of coverage. This exclusion does not apply if You applied for and were issued this policy under guaranteed issue status; if on the date of application for this policy You had at least six (6) months of prior Creditable Coverage; or, if this policy is replacing another Medicare Supplement policy and a six (6) month waiting period has already been satisfied. Evidence of prior coverage or replacement must have been disclosed on the application for this policy. If You had less than six (6) months prior Creditable Coverage, the Preexisting Conditions limitation will be reduced by the aggregate amount of Creditable Coverage. If this policy is replacing another Medicare Supplement policy, credit will be given for any portion of the waiting period that has been satisfied.
